Coaching Session: Planning With The Person In The Context Of The FamilyFamily plays an important part of all our lives. Perspectives, needs, and changes within our family impacts each one of us. When planning with a person with developmental disabilities, it’s important to recognize and support them in the context of their family system. While person-centered planning rightly prioritizes the individual’s voice, hopes, and dreams, we must also navigate the perspectives and needs of the family members who often provide daily support. Siblings, parents and extended family members alike have valuable knowledge about the person, past life experiences, goals, and opportunities. Effective support coordination weaves together both the person’s aspirations and the family’s needs, creating a meaningful plan for life.
This collaborative coaching session provides a forum for Support Coordinators to share resources, collaborate to problem-solve common issues, and enhance skills through facilitated discussion. Each session involves a brief introduction to the topic through use of key best practices, scenarios, case studies, and/or common experiences. Participants will actively participate through guided conversation and sharing of strategies and promising practices.
Participants are expected to actively contribute to the conversation by coming prepared to brainstorm and share resources related to the topic.
Note: This training currently is ONLY open to DDD/Medicaid Approved Support Coordination agencies and Employees of DDD.
In order to assure engagement and active participation, you must use a device with working camera. It is strongly recommended that you use a computer or a laptop with a strong WiFi connection. You must be in a stationary location (e.g. not driving, shopping, working, etc...). Phones and mobile devices are discouraged due to connectivity issues. Please connect with your employer in advance of the session. They should provide you with the technology and appropriate scheduling to successfully complete training required of your job.
To meet the training needs of the entire state, there is a limit of participants per agency for this training.
Certificates: Certificates of Attendance with training title and duration will be provided.
