Coaching Session: Connecting To CommunityAs both best practice and HCBS rule tells us, Medicaid funded services and supports should not be the only means by which people are supported. People with disabilities need to be included in and utilize the resources available to them through their local communities. Further, people should be intentionally included in community opportunities. This goes beyond simply being in the community, but rather, being a part of the community.
Responsibility for this does not rest on one person alone. The entire team plays a role in identifying opportunities, building relationships, and supporting authentic community inclusion. This coaching session will focus on how Support Coordinators and team members are working well together to make this a reality in the lives of the people they support.
Participants are expected to actively contribute to the conversation by coming prepared to brainstorm and share resources related to the topic.
Note: This training currently is ONLY open to Employees of DDD, DDD/Medicaid Approved Support Coordination agencies, and Support Coordination agencies authorized by DDD to move forward with the training required in qualifications.
In order to assure engagement and active participation, you must use a device with working camera. It is strongly recommended that you use a computer or a laptop with a strong WiFi connection. You must be in a stationary location (e.g. not driving, shopping, working, etc...). Phones and mobile devices are discouraged due to connectivity issues. Please connect with your employer in advance of the session. They should provide you with the technology and appropriate scheduling to successfully complete training required of your job.
To meet the training needs of the entire state, there is a limit of participants per agency for this training.
Certificates: Certificates of Attendance with training title and duration will be provided.
